In-home coverage across Wisconsin

We serve members and their caregivers. Our members include older adults and individuals of all ages with disabilities and other long-term care needs.

From our office in Hudson, we support people in large cities, small towns, and rural areas throughout the great state of Wisconsin.

Across Wisconsin and nationwide, we support people just like you – members, caregivers, and authorized representatives who take a direct role in their in-home services. We invite you to learn about some of the ways we can connect as a community. We believe we can achieve more together.
Our history is all about caring for you
Where we come from
Consumer Direct Care Network was founded in 1990 in Missoula, Montana, to provide in-home nursing care. In 1998, self-directed services became an option in Montana. We saw firsthand that people are happiest and healthiest when they receive the care they need in their homes and communities. We began our journey to support programs that allow for greater choice and control for Medicaid participants across the country.
Where we are today
Since then, we have expanded our service models and our range. We support a variety of programs in 14 states across the country.
Our person-centered approach, focus on community-based offices and staff, comprehensive experience, innovative technology solutions, and collaborative relationships make us a leader in the field of self-direction.
We continue to stand upon our caregiving foundation established more than 30 years ago: empowering and supporting people to live the life they want and remain in their homes and communities.

We believe in the philosophy of self-direction. We know people are happiest and healthiest when they have control over their care. Self-direction will always be at the heart of what we do. We will continue to advance self-direction, providing high-quality services to our clients and their caregivers and working to empower and support people to live the life they choose.
